It’s that time of year again for open enrollment into the Union’s Short Term Disability Plan. Your postcard is going out in today’s mail. A copy is attached. If you are in the plan and want to continue to stay in the plan, you DO NOT have to do anything.
If you want to enroll in the plan, please contact the hall for your enrollment form.
If you want to get out of the plan, send something in writing to the hall with your request to cancel.
